How to Apply for Proof of Vaccinations

Please see attached letter from Bishop Anna concerning vaccinations and service attendence. As stated in the letter we WILL NOT by asking for proof of vaccinations. We will be going on the honour system. 

For those of you that are wanting to attend events, eat in restaurants, etc. you will need proof of vaccination. THIS IS NOT THE CARD YOU RECIEVED WHEN YOU GOT YOUR VACCINATIONS. You have to apply for this. 

If someone just wants the paper “passport” they will need their medical card and need to phone 1-833-838-2323 after Labour Day.
